Hemodynamic and autonomic effects of smokeless tobacco in healthy young men.

What was done
Sixteen healthy male habitual snuff users (aged 22 ± 1 years) participated in a randomized, double-blind, placebo-controlled crossover trial evaluating the acute effects of snuff tobacco versus placebo across two separate sessions. Measured parameters included plasma nicotine, catecholamines (epinephrine, norepinephrine), blood pressure, electrocardiogram (heart rate), calf blood flow, peripheral vascular resistance, and muscle sympathetic nerve activity (MSNA).
What was found
Snuff tobacco increased plasma nicotine from 2.8 ± 0.5 ng/ml to 10.4 ± 1.1 ng/ml. Mean blood pressure increased by 10 ± 1 mm Hg and heart rate increased by 16 ± 2 beats/min. Epinephrine concentrations increased by approximately 50%, whereas peripheral vascular resistance, MSNA, and norepinephrine showed no significant change.
Why it matters
It demonstrates that smokeless tobacco delivers rapid systemic nicotine, acting as an acute hemodynamic and autonomic stimulus driven in part by adrenal medullary catecholamine release, without triggering compensatory decreases in sympathetic tone.
Limits
The study is limited by a small sample size (n = 16) confined strictly to young, healthy male habitual users, preventing generalization to females, older individuals, or non-habitual users. Measurements captured only acute laboratory responses rather than long-term cardiovascular risks.
