What was done

Longitudinal observational analysis of a single-center cohort of 60 patients with deficiency of adenosine deaminase 2 (DADA2). The study evaluated disease presentation across inflammatory/vascular, immune dysregulatory, and hematologic phenotypes, as well as outcomes associated with anti-TNF therapy and allogeneic hematopoietic cell transplantation (HCT) using reduced-intensity or non-myeloablative conditioning.

What was found

Clinical features overlapped heavily: 50% of patients had hematologic findings (pure red cell aplasia, neutropenia, or pancytopenia), and immune dysregulation was common despite rare infectious complications. Over 2,026 patient-months on anti-TNF agents, zero strokes occurred, though hematologic and immune manifestations showed variable response. Six patients underwent a total of 10 allogeneic HCT procedures; 3 patients experienced secondary graft failure requiring repeat HCT and unplanned donor cell infusions, but all 6 were alive and successfully stopped anti-TNF therapy at follow-up.

Why it matters

This study documents the broad, overlapping multisystem spectrum of DADA2, confirms the effectiveness of anti-TNF therapy for stroke prevention, and shows that allogeneic HCT can be curative for severe manifestations despite a high incidence of graft instability.

Limits

Single-center uncontrolled cohort design subject to referral bias. The transplant subgroup is very small (6 patients), conditioning regimens varied, and the abstract lacks statistical comparators, detailed follow-up duration for individual phenotypes, and effect size confidence intervals.
