Premature ovarian insufficiency and autoimmune profiles: a prospective case-control study.

A prospective case-control study analyzed blood samples from 66 women with spontaneous premature ovarian insufficiency (POI) and 66 healthy controls to evaluate a panel of autoimmune antibodies, including thyroglobulin antibodies (TGAb) and thyroid peroxidase antibodies (TPOAb). Associations were analyzed using binary logistic regression.
What was found
At least one abnormal autoimmune parameter was detected in 37.9% of women with POI compared to 18.2% of healthy controls (p = 0.045). In multivariable logistic regression, POI was significantly associated with elevated TGAb (adjusted OR 3.586, p = 0.028), elevated TPOAb (adjusted OR 7.496, p = 0.003), and having any abnormal autoimmune parameter (adjusted OR 3.189, p = 0.008).
Why it matters
This study provides prospective evidence linking spontaneous premature ovarian insufficiency with an increased prevalence of thyroid autoimmunity, supporting autoimmune screening—particularly for thyroid disorders—in women presenting with POI.
Limits
The sample size was modest (66 cases and 66 controls). The abstract does not specify all non-thyroid antibodies tested, absolute antibody titers, clinical diagnostic criteria for overt autoimmune disease, or longitudinal outcomes to determine whether antibody positivity led to clinical thyroid dysfunction.
