What was done

Researchers analyzed gait kinematics, hip abduction strength, and self-reported pain in 152 pre-operative adolescent patients clinically diagnosed with acetabular hip dysplasia, femoroacetabular impingement, Legg-Calvé-Perthes disease, or slipped capital femoral epiphysis. Patients with dynamic contralateral pelvic drop ≥ 5.4° during single leg stance (SLS) without excessive ipsilateral trunk lean were categorized into an uncompensated 'pelvic drop' group and compared to 'stable pelvis' patients using Mann-Whitney tests.

What was found

Patients with acetabular dysplasia made up the largest share of the pelvic drop group (46%). The pelvic drop group demonstrated significantly higher self-reported hip pain (p = 0.011), elevated maximum hip abductor moments (p = 0.002), and increased peak coronal power absorption during SLS loading response (p < 0.001). Hip abduction strength did not differ significantly between groups (p = 0.381). Exact group sizes, numerical averages, and effect sizes were not reported in the abstract.

Why it matters

These findings suggest that uncompensated Trendelenburg gait in adolescents increases mechanical loading and passive structural stress on the hip joint independently of muscle weakness. Interventions may need to target mechanical alignment and movement strategies rather than isolated abductor strengthening alone.

Limits

The cross-sectional design precludes establishing whether altered biomechanics cause or result from joint pain and pathology. The cohort combines diverse hip conditions with distinct mechanical etiologies. Specific numerical values, point estimates, and subgroup sample sizes were omitted from the abstract, and long-term joint degradation was not directly measured.
