What was done
This historical public health report analyzed national vital statistics and epidemiological trends in the United States across the 20th century, comparing causes of mortality, life expectancy, and age-specific death proportions between 1900 and 1997, alongside global pandemic mortality estimates.
What was found
US life expectancy increased by 29.2 years during the 20th century. Deaths among children aged <5 years fell from 30.4% of total deaths in 1900 to 1.4% in 1997. In 1900, the top three causes of death (pneumonia, tuberculosis, and diarrhea/enteritis), combined with diphtheria, accounted for one-third of all deaths (40% of which occurred in children <5 years). By 1997, heart disease and cancer caused 54.7% of deaths, while pneumonia, influenza, and HIV accounted for 4.5%. Major disease shocks included the 1918 influenza pandemic (20 million global deaths, 500,000 US deaths in <1 year) and the HIV pandemic (33 million infected, 13.9 million deaths globally at the time of report).
Why it matters
It quantitatively documents the epidemiological shift in the United States from infectious to chronic causes of death over the 20th century, largely driven by gains in child survival.
Limits
The abstract reports aggregated historical vital statistics without specific cohort sample sizes, confidence intervals, or statistical testing. Differences in diagnostic criteria and death certification standards between 1900 and 1997 are not accounted for in the abstract.
