Researchers evaluated a synthetic 26-mer peptide (C6) derived from the invariable region 6 (IR6) of the Borrelia burgdorferi surface antigen VlsE as an ELISA diagnostic target. The assay was tested against serum from mice experimentally infected with three European pathogenic genospecies (B. burgdorferi sensu stricto, B. garinii, and B. afzelii) as well as serum collected from European patients at various clinical stages of Lyme disease.
What was found
Mice infected with any of the three genospecies mounted a strong antibody response to C6. Among human clinical cohorts, anti-C6 antibodies were detected in 20 of 23 patients (87.0%) with culture-confirmed erythema migrans, 14 of 20 patients (70.0%) with late Lyme disease symptoms, and achieved a reported sensitivity of 95.2% in patients with acrodermatitis chronica atrophicans (exact sample size for this group not stated).
Why it matters
Genetic diversity among Borrelia species complicates standard serodiagnosis in regions like Europe where multiple genospecies circulate. Demonstrating that the C6 peptide target is antigenically conserved across all three main pathogenic species suggests it could serve as a universal antigen for Lyme disease ELISA testing.
Limits
The abstract reports small clinical subgroup sizes and does not state the total number of acrodermatitis chronica atrophicans samples tested. Crucially, the abstract provides no data on diagnostic specificity, false-positive rates, or testing against healthy controls or patients with cross-reactive conditions.
