What was done
Prospective cohort study tracking 221 healthy women planning pregnancy across 696 menstrual cycles. The timing of ovulation and the six-day fertile window were estimated using urinary metabolites of estrogen and progesterone.
What was found
The fertile window occurred across a broad range of days in the menstrual cycle. On every day between cycle days 6 and 21, women had at minimum a 10% probability of being in their fertile window. Only about 30% of women had their fertile window fall entirely within the days identified by clinical guidelines (days 10 to 17). Between 4% and 6% of women whose cycles had not resumed were potentially fertile in the fifth week of their cycle.
Why it matters
Standard clinical guidance assuming fertility is confined to days 10 to 17 fails to capture the fertile window for approximately 70% of women. This high unpredictability has direct consequences for both natural contraception and fertility timing.
Limits
The study included 221 women, all of whom were planning a pregnancy, which may limit generalizability. Ovulation was estimated through urinary hormone metabolites rather than direct follicular tracking via ultrasound. Specific participant demographics, cycle regularity baselines, and confidence intervals are not reported in the abstract.
