Substituting dietary saturated for monounsaturated fat impairs insulin sensitivity in healthy men and women: The KANWU Study.

What was done
In the multicenter KANWU study, 162 healthy men and women were randomly assigned to follow a controlled, isoenergetic diet for 3 months containing a high proportion of either saturated fatty acids (SAFA) or monounsaturated fatty acids (MUFA). Within each dietary arm, participants were further randomized to receive either fish oil supplements (3.6 g n-3 fatty acids per day) or placebo. The study assessed changes in insulin sensitivity, insulin secretion, LDL cholesterol, and lipoprotein(a).
What was found
Insulin sensitivity decreased significantly on the SAFA diet (-10%, p = 0.03) and did not change significantly on the MUFA diet (+2%, NS; p = 0.05 for between-diet difference). Insulin secretion was unaffected, and n-3 fatty acid supplementation had no effect on either insulin sensitivity or insulin secretion. The benefit of substituting MUFA for SAFA on insulin sensitivity occurred only among participants with total fat intake below the median (37% of total energy); in this subgroup, insulin sensitivity was 12.5% lower on SAFA and 8.8% higher on MUFA (p = 0.03). LDL cholesterol rose on SAFA (+4.1%, p < 0.01) and decreased on MUFA (-5.2%, p < 0.001), while lipoprotein(a) increased on MUFA by 12% (p < 0.001).
Why it matters
This trial demonstrates that shifting fat quality from saturated to monounsaturated fat improves insulin sensitivity in healthy adults, though high total fat intake blunts this benefit.
Limits
The abstract does not report baseline metabolic characteristics, compliance details, exact dropout rates, or clinical endpoints beyond 3 months. The finding that benefits disappear above 37% total energy intake relies on a median-split subgroup analysis rather than a pre-specified, separately randomized comparison.
