This narrative review synthesized findings from post-mortem human donor eye histology and functional psychophysical studies to evaluate the relative rates and spatial topography of rod versus cone photoreceptor loss during normal retinal aging and age-related maculopathy (ARM).
What was found
In donor eyes with grossly normal maculae, foveal cone numbers remained stable while parafoveal rods declined by 30% over adulthood, a pattern extending into early ARM. In exudative ARM, photoreceptors surviving over disciform scars were predominantly cones, while rods declined sharply in association with thick subretinal pigment epithelium deposits. Functional tests showed a greater loss of scotopic (rod-mediated) sensitivity compared to photopic (cone-mediated) sensitivity in normal aging and early ARM. The topographic pattern of rod loss matched the distribution of early ARM lesions but did not match the distribution of lipofuscin-associated fundus autofluorescence.
Why it matters
Identifying selective parafoveal rod vulnerability over foveal cones clarifies the spatial progression of early ARM and highlights scotopic functional testing as a sensitive marker for early disease detection.
Limits
