Researchers re-examined patient SM, an individual with rare bilateral amygdala damage known to have impaired recognition of fear from facial expressions. Visual fixation patterns were assessed during free viewing of facial expressions, and fear recognition performance was evaluated with and without explicit instructions to look at the eye region of faces.

What was found

The abstract reports no numerical values. It describes that patient SM failed to spontaneously fixate on the eye region across all facial expressions. Because the eyes are the primary visual cue for identifying fear, this lack of spontaneous fixation selectively compromised fear recognition. When explicitly instructed to direct gaze to the eyes, SM's recognition of fearful expressions normalized completely.

Why it matters

This study demonstrates a visual attentional mechanism underlying the amygdala's role in emotion perception, showing that fear recognition deficits can stem from absent eye fixation rather than an inability to process fear per se, and suggests potential targeted gaze-rehabilitation strategies.

Limits

The study is limited to a single patient (n = 1) with rare bilateral amygdala lesions, limiting statistical power and direct generalizability to broader clinical populations. Long-term retention of instructed gaze behavior and real-world dynamic social interactions were not evaluated.
