What was done

A prospective comparative study was conducted in 89 patients undergoing elective cervical or lumbar spinal surgery assigned to rooms on either the "bright" or "dim" side of the same hospital unit. Daily sunlight intensity in rooms was recorded. Psychologic questionnaires were administered on the day after surgery and at discharge, and total analgesic consumption was converted to standard morphine equivalents per hour.

What was found

Rooms on the bright side received 46% higher-intensity sunlight on average (p = .005). Patients exposed to higher sunlight intensity experienced less perceived stress (p = .035), marginally less pain (p = .058), consumed 22% less analgesic medication per hour (p = .047), and incurred 21% lower analgesic costs (p = .047). Increasing age quartile was also negatively correlated with analgesic use (p < .001), but the reduction in analgesic use in bright rooms remained consistent across all age quartiles.

Why it matters

Physical environmental factors in inpatient facilities, particularly access to natural sunlight, may meaningfully decrease postoperative pain perception, analgesic requirements, and hospital costs.

Limits

Room allocation was non-randomized, introducing potential confounding. The study was conducted at a single site with a relatively small sample size (n = 89). The abstract does not specify whether staff or patients were blinded or how seasonal variations in sunlight were controlled.
