What was done

Authors searched PubMed, SportDiscus, Embase (1997–2006), reference lists, and consulted experts to identify studies examining cardiac troponin T (cTnT) release following endurance exercise. A random-effects meta-analysis was conducted across 26 studies (1,120 cases). Meta-regressions evaluated age, gender, body mass, exercise mode, and event duration as potential moderators.

What was found

Postexercise cTnT exceeded the assay detection limit in 47% of participants (95% CI = 39–56%). Detection of postexercise cTnT was roughly half as frequent after cycling compared to running events (27% vs. 52%, P = 0.042). Detection decreased slightly as event duration increased (P = 0.022) and as mean body mass decreased (P = 0.0033). Detection was slightly higher in cohorts with more males (P = 0.028) and was not significantly affected by age (P = 0.309).

Why it matters

This review establishes that elevated cTnT is present in nearly half of endurance athletes post-event, particularly heavier runners in shorter events. This benchmark helps clinicians avoid misinterpreting benign postexercise biomarker release as acute myocardial infarction.

Limits

The abstract pools small observational cohorts without non-exercise control groups. Exact assay generations, detection thresholds, sampling time points, and long-term clinical outcomes were not detailed in the abstract.
