What was done

Researchers quantified pigmented neuron density in single 7 µm haematoxylin and eosin-stained caudal substantia nigra sections across human post-mortem cases. The pars compacta was divided into ventral and dorsal tiers, each subdivided into three subregions. The study included 36 neurologically normal controls of varying age, 20 Parkinson's disease (PD) cases, 7 incidental Lewy body cases, 15 striatonigral degeneration cases, and 14 Steele-Richardson-Olszewski syndrome cases.

What was found

In controls, pigmented neurons declined linearly at 4.7% per decade, with relative sparing of the lateral ventral tier (2.1% loss per decade) compared to the dorsal tier (6.9%). In PD, neuron loss was exponential (45% in the first decade) and selectively targeted the lateral ventral tier (average loss 91%), medial ventral tier (71%), and dorsal tier (56%). Incidental Lewy body cases showed cell loss confined to the lateral ventral tier (average 52%). Estimated cell loss at clinical symptom onset was 68% in the lateral ventral tier and 48% in the caudal nigra overall. Striatonigral degeneration showed 21% greater dorsal tier loss than PD, while Steele-Richardson-Olszewski syndrome involved the medial nigra and spared the lateral nigra.

Why it matters

The study shows that the regional distribution and rate of substantia nigra neuronal loss in Parkinson's disease are distinct from normal aging, indicating that the disease is not simply accelerated age-related attrition.

Limits

Evaluations relied on single 2D tissue sections from a specific caudal level rather than full 3D stereological volumetric cell counting. Subgroup sample sizes were modest (e.g., n=7 for incidental Lewy body cases), and cross-sectional post-mortem data were used to estimate longitudinal temporal dynamics and disease onset thresholds.
