Moderate exercise improves depression parameters in treatment-resistant patients with major depressive disorder.

What was done
Out of 150 screened individuals with treatment-resistant major depressive disorder (taking combined pharmacotherapy for 9–15 months without remission), 33 patients were randomized to usual pharmacotherapy alone (n = 11) or usual pharmacotherapy plus aerobic exercise (n = 22). The exercise intervention consisted of home-based 30–45 min/day walks, 5 days/week for 12 weeks, with 1 walk per week supervised.
What was found
The exercise group showed statistically significant improvements compared to baseline and the control group (p < 0.05) on depression and functioning scales (lower HAMD17, BDI, and CGI-S; higher GAF). At the end of the study, 0% of the control group achieved response or remission, whereas 21% of the exercise group showed response and 26% showed remission, though these categorical differences were not statistically significant.
Why it matters
It suggests that an accessible, home-based walking regimen can serve as a beneficial adjunctive intervention for patients with difficult-to-treat depression who have not responded to standard pharmacotherapy.
Limits
The randomized sample size was very small (n = 33), limiting statistical power to demonstrate significant differences in clinical response and remission rates. The majority of walking sessions were unsupervised, and blinding procedures were not described.
