Red blood cell omega-3 fatty acid levels and neurocognitive performance in deployed U.S. Servicemembers.

What was done
Cross-sectional study of 78 U.S. servicemembers deployed to Iraq who screened positive for mild-to-moderate depression on the PHQ-9. Dried blood spot samples measured the HS-Omega-3 Index (EPA + DHA). Participants completed psychosocial questionnaires (Pittsburgh Sleep Quality Index, Epworth Sleepiness Scale, Zung Depression and Anxiety scales, Combat Experiences scale) and computerized neurocognitive testing measuring five domains: processing speed, complex attention, reaction time, cognitive flexibility (CF), and executive function (EF). Linear trends across HS-Omega-3 Index quartiles were evaluated using ANOVA contrasts.
What was found
The mean HS-Omega-3 Index was 3.5 ± 0.7%. Omega-3 index was not significantly associated with depression, anxiety, or sleep scores. However, higher omega-3 levels were directly associated with better CF (P < 0.02) and EF (P < 0.01). In the subgroup reporting poor sleep quality (81%, n = 63), servicemembers with omega-3 levels above the mean had significantly higher EF and CF scores compared to those below the mean (P = 0.005).
Why it matters
The findings suggest that omega-3 fatty acid status is linked to executive cognitive performance in high-stress operational environments and may provide resilience against sleep-related cognitive deficits.
Limits
The cross-sectional design cannot establish causality. The sample size was small (n = 78) and restricted to deployed personnel with mild-to-moderate depression, limiting generalizability to healthy or non-military populations. Unmeasured confounders such as specific dietary intake or head trauma were not detailed in the abstract.
