What was done

Ten normozoospermic male volunteers participated in a longitudinal time-course study involving two 15-minute Finnish sauna sessions per week for 3 months at 80–90°C. Semen parameters, sex hormones, sperm chromatin structure, sperm apoptosis, and heat stress/hypoxia gene expression were measured at baseline (T0), immediately following 3 months of sauna exposure (T1), and at 3 months (T2) and 6 months (T3) after sauna cessation. Data were analyzed using paired Student's t-tests.

What was found

After 3 months of sauna exposure (T1), sperm count and motility were significantly impaired (P < 0.001), while sex hormone levels showed no significant change. Percentage of sperm with normal histone-protamine substitution decreased from 78.7 ± 4.5% to 69.0 ± 4.1%, normal chromatin condensation decreased from 70.7 ± 4.7% to 63.6 ± 3.3%, and normal mitochondrial function decreased from 76.8 ± 4.9% to 54.0 ± 6.1%. Genes responsive to heat stress and hypoxia were strongly up-regulated. All alterations returned to baseline by 6 months after sauna cessation (T3).

Why it matters

This study provides evidence that regular sauna-induced scrotal hyperthermia induces substantial but completely reversible impairments in human sperm quality, DNA packaging, and mitochondrial function.

Limits

The sample size was very small (n = 10), and the study lacked an unexposed control group. It enrolled only normozoospermic individuals, leaving effects on men with baseline abnormal sperm parameters unexamined. Absolute baseline and post-exposure values for sperm count and motility were not reported in the abstract.
