The authors conducted a coordinate-based meta-analysis using Effect Size Signed Differential Mapping (ES-SDM) across 19 functional neuroimaging studies of symptom provocation in posttraumatic stress disorder (PTSD). The dataset included 274 PTSD patients across all 19 studies and 145 trauma-exposed control participants from 13 studies. Analyses evaluated neural responses to trauma-related stimuli versus neutral conditions and group differences between PTSD patients and controls.
What was found
The abstract reports directional activation patterns without numerical effect sizes, test statistics, or p-values: - PTSD patients showed significant activation in the midline retrosplenial cortex and precuneus during trauma-related stimuli compared to controls and neutral conditions. - PTSD patients exhibited hyperactivation in the pregenual/anterior cingulate gyrus and bilateral amygdala during trauma-relevant versus neutral stimuli. - PTSD patients demonstrated significantly less activation than controls in sensory association areas, including the bilateral temporal gyri and extrastriate area.
Why it matters
Established neurocircuitry models of PTSD focus on amygdala hyperreactivity and medial prefrontal hypoactivation, which account for emotional dysregulation but do not fully explain intrusive autobiographical memory flashbacks. Identifying retrosplenial cortex and precuneus involvement links symptom provocation directly to neural circuits governing autobiographical memory retrieval and priming.
Limits
Primary neuroimaging studies had very small sample sizes (averaging approximately 14 PTSD patients per study). Six of the 19 studies lacked control cohorts, and no non-trauma-exposed comparison groups were evaluated. The abstract provides no quantitative effect sizes, coordinate locations, or metrics of between-study heterogeneity.
