Systematic review of literature indexed in PubMed from inception to May 2013 on maternal alcohol consumption during lactation. The authors assessed prevalence, milk ejection reflex and yield, alcohol pharmacokinetics in lactating women and nursing infants, and reported infant outcomes across 41 included publications.
What was found
Approximately half of lactating women in Western countries report alcohol intake. Alcohol concentrations in breast milk closely track maternal blood levels. Nursing infants receive an estimated 5-6% of the weight-adjusted maternal dose and clear alcohol at roughly half the adult metabolic rate. Alcohol temporarily inhibits the milk ejection reflex, reducing milk yield. Reported infant behavioral changes are minor and contradictory across studies, with no clear evidence of adverse effects from occasional maternal drinking.
