Habitual alcohol consumption associated with reduced semen quality and changes in reproductive hormones; a cross-sectional study among 1221 young Danish men.

What was done

Researchers conducted a cross-sectional study of 1,221 young Danish men (aged 18–28 years) undergoing compulsory medical examinations for military service between 2008 and 2012. Alcohol intake was evaluated across three metrics: intake in the preceding week, typical weekly consumption, and binge drinking episodes (>5 units/day) in the past 30 days. Outcomes measured included semen quality parameters (volume, concentration, total count, motility, and morphology) and serum reproductive hormone levels (FSH, LH, testosterone, SHBG, estradiol, free testosterone, and inhibin B).

What was found

Increasing habitual weekly alcohol consumption was negatively associated with sperm concentration, total sperm count, and the percentage of morphologically normal spermatozoa. Adverse associations were detectable starting at 5 units/week and were most pronounced at >25 units/week. Men consuming >40 units/week had a 33% reduction (95% CI 11% to 59%) in sperm concentration compared to those consuming 1–5 units/week. Recent alcohol intake in the preceding week was significantly associated with increased serum free testosterone. Binge drinking was not independently associated with semen quality.

Why it matters

The study indicates that even modest habitual alcohol intake (>5 units/week) is associated with reduced semen quality in young men, highlighting potential dose-dependent reproductive toxicity from regular alcohol consumption.

Limits

The cross-sectional design cannot determine causality or account for temporal changes in spermatogenesis (which takes ~74 days). Alcohol consumption was self-reported and subject to recall bias. The cohort consisted exclusively of young Danish military conscripts, which may limit generalizability to older or clinically infertile populations.
