What was done

In the Study of Women's Health Across the Nation (SWAN), a prospective multiracial and multiethnic cohort of 3302 women across 7 US sites was followed from 1996 to 2013 over a median of 13 visits. Analyses included 1449 women with frequent vasomotor symptoms (VMS; hot flashes or night sweats on at least 6 days in the previous 2 weeks). The study assessed total VMS duration, post-final menstrual period (post-FMP) persistence, and associated risk factors.

What was found

Median total VMS duration was 7.4 years. Among 881 women with an observable FMP, median post-FMP persistence was 4.5 years. Women who were premenopausal or early perimenopausal at symptom onset had the longest median total duration (>11.8 years) and post-FMP persistence (9.4 years), compared with those postmenopausal at onset (median 3.4 years). African American women had the longest total duration (median 10.1 years). Younger age, lower education, higher perceived stress, symptom sensitivity, depressive symptoms, and anxiety at onset were also associated with longer symptom duration.

Why it matters

These findings show that frequent vasomotor symptoms typically last over 7 years—far longer than previously assumed—and persist for nearly half a decade after the final menstrual period, providing realistic timelines for patient counseling and menopause management.

Limits

Vasomotor symptoms were based on self-report over a 2-week recall window at periodic visits rather than continuous objective monitoring. Post-FMP persistence could only be determined in the subgroup of 881 women with an observable final menstrual period. Observational associations with affective and sociodemographic factors do not establish causality.
