Association between phthalate metabolites and biomarkers of reproductive function in 1066 Chinese men of reproductive age.

In a cross-sectional study of 1,066 Chinese men of reproductive age, researchers measured 14 urinary phthalate metabolites alongside reproductive biomarkers. Biomarkers included serum insulin-like factor 3 (INSL3), total testosterone, free testosterone, free androgen index, luteinizing hormone, sperm motility, sperm morphology, sperm acrosin activity, and sperm DNA fragmentation index (DFI). Statistical associations were assessed with unadjusted phthalate concentrations as independent variables and urinary creatinine as a separate covariate.
What was found
The abstract reports directions of association without numerical values, confidence intervals, or p-values. Serum INSL3 was negatively associated with mono-2-ethylhexyl phthalate (MEHP) and %MEHP. Sperm acrosin activity was negatively associated with mono-n-butyl phthalate (MBP), mono-isobutyl phthalate (MiBP), MEHP, and %MEHP. MBP and MiBP were also negatively associated with total testosterone, free androgen index, free testosterone, luteinizing hormone, and normal sperm morphology, and positively associated with DNA fragmentation index. %MEHP was negatively associated with sperm motility.
Why it matters
This study provides early epidemiological evidence linking environmental phthalate exposure to reduced Leydig cell function (via INSL3) and impaired acrosin activity, identifying potential specific mechanisms of plasticizer-induced male reproductive toxicity.
Limits
The cross-sectional design cannot establish causality or temporality. The abstract omits all numerical effect sizes and uncertainty estimates. Single-spot urinary phthalate measurements may not reflect long-term exposure during the full spermatogenic cycle. The population was restricted to Chinese men of reproductive age, limiting generalizability to other demographics.
