Mortality Risk of Hypnotics: Strengths and Limits of Evidence.

The author reviewed literature published over a 30-year period to identify descriptive epidemiologic studies assessing mortality risk associated with hypnotics and sedative-anxiolytics. The review evaluated 34 studies reporting risk ratios, odds ratios, or hazard ratios alongside plausible mechanisms of harm.

Of the 34 identified studies, 24 reported statistically significant (p < 0.05) excess mortality associated with hypnotics, including all 14 of the largest studies. Zero studies found that hypnotics prolonged life. No pooled numeric effect estimates or confidence intervals were provided in the abstract due to study heterogeneity.
Why it matters
This review highlights consistent observational evidence linking hypnotic medications to increased mortality risk, supporting caution when prescribing sedatives.
Limits
The findings are derived from observational studies vulnerable to confounding by indication, misclassification of hypnotic drugs, unmeasured genetic factors, missing dosage information, and exposure mixing during extended follow-ups. Study heterogeneity prevented formal meta-analysis.
