The effects of classic ketogenic diet on serum lipid profile in children with refractory seizures.

What was done

In an open-label trial, 33 children with refractory epilepsy were treated with a classic ketogenic diet and followed for 6 months. Serum lipid profiles (triglycerides, total cholesterol, LDL, and HDL) were evaluated at baseline, 3 months, and 6 months alongside clinical seizure frequency.

What was found

Seizure frequency was reduced in 63% of children (2/33 became seizure-free and 19/33 experienced a greater than 50% reduction). At 6 months, all measured lipid parameters showed statistically significant median increases (all P < 0.001): - Triglycerides rose from 84 to 180 mg/dl - Total cholesterol rose from 180 to 285 mg/dl - LDL rose from 91 to 175 mg/dl - HDL rose from 51 to 58 mg/dl

Why it matters

Although the classic ketogenic diet is effective for pediatric refractory epilepsy, it causes marked short-term hypercholesterolemia and hypertriglyceridemia, reinforcing the need for regular lipid monitoring during therapy.

Limits

The study was open-label and lacked a control group. The sample size was small (33 children), follow-up was limited to 6 months, and long-term cardiovascular outcomes or resolution after diet discontinuation were not evaluated. Specific dietary ratios and intermediate 3-month lipid values were not reported in the abstract.
