Online Information About Harmful Tobacco Constituents: A Content Analysis.

What was done
A systematic content analysis was conducted in September 2015 to evaluate online communication regarding tobacco constituents. A search using tobacco and constituent terms yielded 226 relevant websites. Two coders independently evaluated source type, target audience, reading level, constituent information, tobacco product type, health effects, and emotional valence, with high interrater reliability across a 20% sample overlap (κ = .83, p < .001).
What was found
The mean reading grade level was 8.2 (SD = 2.8), with 81.7% of websites written above a sixth-grade reading level. Information was predominantly qualitative narrative (93%), with 48.2% presenting quantitative data. The most frequently mentioned constituents were nicotine (59.3%) and nitrosamines (28.8%), and cancer was the most cited health effect (51.3%). Nearly a quarter (23%) of websites did not explicitly state that tobacco constituents or products cause adverse health effects.
Why it matters
As regulators require public disclosure of tobacco constituents, communicating risks clearly to the public is critical. This study shows existing online materials often require high reading literacy and fail to clearly connect chemical constituents to health harms.
Limits
The study is a cross-sectional snapshot from September 2015 and may not reflect current online search results or modern digital formats. It did not directly measure consumer comprehension, risk perception, or behavioral outcomes.
