What was done

Researchers measured the cerebral metabolic rate of glucose and lactate using the Kety-Schmidt method during the acute phase (days 0–7 post-injury) in patients with moderate-to-severe traumatic brain injury (TBI). Frontal-temporal cognitive processing was assessed at 6 and 12 months post-injury. Regression models evaluated whether acute metabolic measures predicted cognitive status at each timepoint and cognitive recovery (the score difference between 6 and 12 months).

What was found

Acute metabolic measures did not significantly predict static frontal-temporal cognition at 6 or 12 months post-injury. However, average glucose and lactate metabolism significantly predicted cognitive recovery between 6 and 12 months, accounting for 23% and 22% of the variance, respectively. Maximum glucose metabolism (but not maximum lactate metabolism) was an inverse predictor of recovery, explaining 23% of the variance. A combined centered average of glucose and lactate metabolism accounted for 22% of the recovery variance. Exact sample sizes and regression coefficients were not reported in the abstract.

Why it matters

This study links acute post-injury cerebral energy metabolism directly to long-term frontal-temporal cognitive recovery, supporting the concept that targeting brain substrate availability early after TBI may influence neural repair.

Limits

The abstract omits the sample size, baseline patient characteristics, and exact statistical coefficients. The findings are strictly observational/correlational, metabolic measurements were constrained to the first week post-injury, and findings may not generalize across milder brain injuries.
