Six patients with idiopathic guttate hypomelanosis (IGH) and six healthy controls were enrolled. Melanin content was evaluated in skin sections and cultured melanocytes. Senescence was assessed in lesional skin compared to control skin by measuring mRNA and protein expression of senescence markers p16, hp1, and p21.

What was found

Cultured melanocytes from IGH patients were larger with small, retracted dendrites and had greater melanin accumulation compared to controls. Lesional skin of IGH patients showed significantly higher mRNA and protein expression of p16, p21, and hp1 than healthy controls. Why it matters

These findings suggest that melanocyte senescence and impaired melanosome transfer to keratinocytes may drive the formation of hypopigmented macules in IGH.
