What was done

The study evaluated 2,777 elderly participants from the French Three-City Study followed over a 12-year period. Refined carbohydrate intake was estimated by quantifying dietary glycemic load. Proportional hazards regression models were used to assess associations between glycemic load and incident dementia and Alzheimer's disease, as well as the interaction with APOE-ε4 carrier status, adjusting for potential confounders.

What was found

After adjustment for potential confounders, high afternoon-snack glycemic load was associated with increased dementia and Alzheimer's disease risk in APOE-ε4 carriers (hazard ratio = 1.27, 95% CI [1.03-1.56]). Numerical estimates for non-carriers or overall daily glycemic load were not reported in the abstract.

Why it matters

This paper provides long-term epidemiological evidence linking high glycemic load intake to elevated dementia risk specifically in genetically vulnerable APOE-ε4 carriers.

Limits

The observational design cannot establish causality and is subject to residual confounding.
