What was done

Authors conducted a systematic review and meta-analysis of randomized controlled trials searching MEDLINE, EMBASE, CENTRAL, and PsycInfo through August 2020. Inclusion required RCTs evaluating dance interventions in older adults aged 55 years or older (healthy or with mild cognitive impairment [MCI]) reporting cognitive function outcomes categorized by the DSM-5 Neurocognitive Framework. Meta-analyses used RevMan 5.3 and evidence certainty was evaluated via GRADE.

What was found

Eleven RCTs (1,412 total participants; 7 in healthy older adults, 4 in MCI) were analyzed. Dance sessions ranged from 35 to 60 minutes, 1 to 3 times per week, for 3 to 12 months. Dance produced improvements in global cognition on the Mini Mental State Examination (MD = 1.58, 95% CI 0.21 to 2.95; moderate certainty) and memory on the Wechsler Memory Test (MD = 3.02, 95% CI 1.38 to 4.65; low certainty). No significant effects were seen on the Trail Making Test-A for complex attention (MD = 3.07, 95% CI -0.81 to 6.95; high certainty) or Trail Making Test-B for executive function (MD = -4.12, 95% CI -21.28 to 13.03; moderate certainty). Subgroup analyses showed no consistently greater effects in MCI.

Why it matters

This review provides quantitative evidence that dance interventions yield modest improvements in global cognition and memory for older adults. However, it clarifies that broad claims of enhancement across other specific cognitive domains, such as complex attention and executive function, are currently unsupported.

Limits

The included trials exhibited substantial variability in dance style, duration (3-12 months), and frequency. Only four studies evaluated adults with MCI. Confidence intervals for executive function and complex attention crossed null. Evidence for language was uncertain, and no studies evaluated social cognition or perceptual-motor function. The review could not establish an optimal exercise dose or determine if dance outperforms other forms of exercise.
