Eighty-two patients with MRI evidence of cerebral small vessel disease performed an effort-based decision-making task involving choices to accept or reject monetary rewards in exchange for physical effort (hand grip force). Choices and reaction times were analyzed using a drift diffusion model. White matter tract integrity was assessed using diffusion-weighted MRI and tract-based spatial statistics to identify structural correlates of apathy while adjusting for depression.
What was found

Patients with apathy accepted significantly fewer task offers, demonstrated blunted responsiveness to low rewards, and showed significant aversion to exerting high effort. The drift rate parameter was associated with both apathy and white matter alterations, whereas depression was characterized by an increased decision boundary.
Why it matters

Identifying distinct computational decision parameters and white matter networks offers objective markers for monitoring disease progression and evaluating targeted interventions.
Limits
