What was done

Researchers tracked 1,497 fully vaccinated healthcare workers at a medical center in Israel who were evaluated for SARS-CoV-2 via RT-PCR after symptoms or exposure. Among 39 documented breakthrough cases, a matched case-control analysis compared peri-infection neutralizing antibody titers (collected within one week prior to detection) to 4–5 uninfected vaccinated controls per case using generalized estimating equations. Genomic sequencing, antigen-detecting rapid diagnostic tests (Ag-RDT), and N gene cycle threshold (Ct) values were also evaluated.

What was found

Neutralizing antibody titers in breakthrough cases during the peri-infection period were significantly lower than in matched controls (case-to-control ratio, 0.361; 95% CI, 0.165 to 0.787). Higher neutralizing titers were associated with higher Ct values (lower infectivity). The B.1.1.7 (Alpha) variant accounted for 85% of sequenced samples. High viral loads (Ct < 30) were observed in 74% of cases, but only 17 of these (59%) tested positive on concurrent Ag-RDT. Most infections were mild or asymptomatic, 19% had persistent symptoms beyond 6 weeks, and zero secondary infections were documented.

Why it matters

This study provides early real-world evidence that lower circulating neutralizing antibody titers serve as a correlate of risk for SARS-CoV-2 breakthrough infection and correlate with higher viral load post-vaccination.

Limits

The analysis included only 39 breakthrough cases from a single healthcare worker population. Testing focused primarily on symptomatic or exposed individuals rather than universal routine screening, meaning some asymptomatic infections may have been missed. Results reflect the B.1.1.7 variant and may differ for subsequent viral lineages.
