The authors synthesized literature examining the role of gut microbiota and butyrate in type 2 diabetes (T2D) and prediabetes. The review evaluated mechanisms of action (colonocyte fuel, histone deacetylase inhibition, G-protein coupled receptor signaling) and examined human strategies to increase intestinal butyrate, including supplementation with butyrate-producing bacteria, dietary fiber, and fecal microbiota transplantation.

What was found

The abstract reports that a reduction in butyrate-producing bacteria is consistently observed across human cohorts with prediabetes and T2D. Interventions to raise intestinal butyrate levels have shown short-term benefits in humans; however, no specific quantitative effect sizes are provided, and no long-term improvements in glycemic control have been reported to date.

Why it matters

Targeting intestinal butyrate via prebiotics, probiotics, or microbiome transplants is a widely discussed therapeutic avenue for metabolic disease, but this review clarifies that durable glycemic control has yet to be demonstrated clinically.

Limits
