What was done

Cross-sectional analysis evaluating the relationship between red blood cell (RBC) omega-3 index (O3I; membrane EPA and DHA content) and red blood cell distribution width (RDW). From a clinical laboratory database of >45,000 individuals, 25,485 healthy adults without inflammation or anemia who had complete data for O3I, RDW, high-sensitivity C-reactive protein (CRP), BMI, age, and sex were analyzed using multivariable-adjusted models.

What was found

RDW was significantly and inversely associated with O3I in both males and females before and after adjusting for sex, age, BMI, and CRP (both p < 0.00001). A significant sex-by-O3I interaction was observed (p < 0.00066), showing an especially steep inverse slope in females with an O3I ≤ 5.6%. Specific effect sizes (such as regression coefficients or mean RDW values) were not reported in the abstract.

Why it matters

Elevated RDW and low omega-3 levels are both independent predictors of all-cause mortality; these findings show that an O3I > 5.6% is linked with lower RDW variation in healthy adults.

Limits

The cross-sectional design cannot determine whether omega-3 intake directly lowers RDW.
