Nicotinamide Adenine Dinucleotide Augmentation in Overweight or Obese Middle-Aged and Older Adults: A Physiologic Study.

What was done
Thirty overweight or obese adults aged 45 years or older were randomized in a 2:1 ratio to receive either MIB-626 (two 500 mg tablets twice daily of microcrystalline beta-nicotinamide mononucleotide) or placebo for 28 days. The study assessed safety, circulating NAD and metabolome levels, body weight, liver, muscle, and intra-abdominal fat, insulin sensitivity, blood pressure, lipids, physical performance (muscle strength, fatigability, aerobic capacity, stair-climbing power), and muscle bioenergetics.
What was found
Adverse events were similar between groups, and MIB-626 substantially increased circulating concentrations of NAD and its metabolites. Compared to placebo, the MIB-626 group experienced statistically significant reductions in body weight (difference -1.9 kg [95% CI: -3.3, -0.5], P = .008), diastolic blood pressure (difference -7.01 mmHg [95% CI: -13.44, -0.59], P = .034), total cholesterol (difference -26.89 mg/dL [95% CI: -44.34, -9.44], P = .004), LDL cholesterol (difference -18.73 mg/dL [95% CI: -31.85, -5.60], P = .007), and non-HDL cholesterol. Changes in muscle strength, fatigability, aerobic capacity, stair-climbing power, insulin sensitivity, and hepatic and intra-abdominal fat did not differ significantly between groups.
Why it matters
This randomized trial demonstrates that oral beta-nicotinamide mononucleotide can safely augment circulating NAD levels in humans and confers short-term improvements in select lipid and cardiometabolic markers in middle-aged and older adults.
Limits
The study was small (30 participants) and brief (28 days). It was restricted to overweight or obese adults aged 45 and older, and the intervention showed no detectable effects on insulin sensitivity, regional fat distribution, or functional physical performance measures.
