Relationship of Daily Step Counts to All-Cause Mortality and Cardiovascular Events.

What was done
A systematic review and dose-response meta-analysis of 12 studies comprising 111,309 general population individuals evaluated associations between objectively measured daily step count metrics and primary outcomes of all-cause mortality and incident cardiovascular disease (CVD). Electronic databases were searched from inception to October 2022, and dose-response analyses were conducted using generalized least squares and random-effects models.
What was found
Compared with a reference level of 2,000 steps/d, significant risk reductions were observed at 2,517 steps/d for all-cause mortality (adjusted HR [aHR]: 0.92; 95% CI: 0.84-0.999) and 2,735 steps/d for incident CVD (aHR: 0.89; 95% CI: 0.79-0.999). Additional steps yielded nonlinear risk reductions with optimal thresholds at 8,763 steps/d for all-cause mortality (aHR: 0.40; 95% CI: 0.38-0.43) and 7,126 steps/d for incident CVD (aHR: 0.49; 95% CI: 0.45-0.55). Moving from low to intermediate or high cadence provided additional independent mortality risk reductions. Dose-response curves did not differ by sex, but hip-worn accelerometers showed more pronounced risk reductions than pedometers and wrist-worn devices.
Why it matters
These findings provide concrete, easily communicated minimum (~2,600-2,800 steps/day) and optimal (~7,100-8,800 steps/day) physical activity targets for clinical counseling and public health prescriptions.
Limits
The underlying studies were observational cohorts, leaving potential for residual confounding and reverse causation (e.g., sicker individuals taking fewer steps). Results varied significantly by the measurement device and wear location (hip vs. wrist vs. pedometer). The abstract does not report average follow-up duration, participant age ranges, or changes in step volume over time.
