What was done
A systematic review and random-effects meta-analysis was conducted according to PRISMA guidelines, searching PubMed, Scopus, EMBASE, Web of Science, Cochrane, and ClinicalTrials.gov through May 27, 2023. The authors included randomized controlled trials evaluating the analgesic effects and adverse events of collagen peptides or hydrolyzed collagen versus placebo in patients with knee osteoarthritis. Study quality was appraised using Cochrane RoB 2.0 and evidence certainty was evaluated with GRADE.
What was found
Four randomized controlled trials with a total of 507 patients were analyzed. Collagen peptides produced a statistically significant reduction in pain compared with placebo (standardized mean difference: -0.58; 95% CI: -0.98 to -0.18; p = 0.004; I2 = 68%; GRADE quality: moderate). The difference in adverse event risk between collagen peptide and placebo groups was not statistically significant (odds ratio: 1.66; 95% CI: 0.99 to 2.78; p = 0.05; I2 = 0%; GRADE quality: very low).
Why it matters
This review suggests that oral collagen peptides may provide moderate pain relief in knee osteoarthritis without a significant increase in reported adverse events.
Limits
The total evidence base is small, comprising only 4 trials and 507 patients. All included trials were judged to have a high risk of bias under Cochrane RoB 2.0. There was substantial heterogeneity for the primary pain outcome (I2 = 68%), the evidence quality for safety was rated very low, and the abstract provides no data on long-term outcomes or structural joint changes.
