What was done

Retrospective review of clinical and autopsy records for 19 sudden cardiac deaths among 1,606,167 medically screened US Air Force recruits (90% male, aged 17 to 28 years) during a 42-day basic training period between 1965 and 1985. Autopsy records of 32 non-sudden, non-cardiac deaths occurring in the same period were also evaluated.

What was found

Of the 19 sudden cardiac deaths, 16 occurred in males with identifiable underlying structural heart disease, while 3 cases had no identifiable anatomic cause. Strenuous physical exertion was associated with 17 of the 19 sudden deaths (0.017 deaths per 50,000 exercise-hours). Myocarditis was the most common underlying etiology. Among 32 non-sudden, non-cardiac deaths, structural heart disease was present in only 2 individuals.

Why it matters

This 20-year surveillance study establishes that sudden cardiac death is extremely rare among healthy, screened young adults, is predominantly triggered by heavy exertion, and is frequently driven by myocarditis.

Limits

The study is retrospective and relies on the consistency of autopsy reporting across two decades. Findings from young, medically pre-screened, 90% male military recruits during a brief 42-day training window may not generalize to women, older adults, or the broader unscreened civilian population.
