A single-blind controlled trial evaluated the residual inhibition effect of tailor-made notched music training (TMNMT) compared to a placebo music control in 44 individuals with tinnitus (22 participants per group). Electrophysiological and behavioral assessments were conducted across three states: eyes closed pre-stimulus, during stimulus, and post-stimulus. Outcomes were measured using behavioral tests of tinnitus suppression, EEG power spectral density across delta, theta, alpha, beta, and gamma frequency bands, and EEG microstate dynamics across microstate classes A through D.

Tinnitus symptom intensity was significantly positively correlated with the duration of microstate B across five evaluated conditions.
Why it matters
This study provides mechanistic evidence that acoustic residual inhibition via notched music alters specific cortical oscillatory and microstate dynamics. These EEG markers may serve as objective functional correlates for evaluating transient tinnitus suppression.
