Investigation of exacerbating factors for postpartum hair loss: a questionnaire-based cross-sectional study.

What was done

A questionnaire-based cross-sectional study evaluated women who delivered at two clinical facilities and completed a survey 10 to 18 months postpartum. The questionnaire assessed baseline characteristics, pregnancy and delivery details, childcare practices, and the presence or absence of postpartum hair loss. Logistic regression analyses were performed to identify independent predictors of postpartum hair loss.

What was found

A total of 331 responses were analyzed (21.0% response rate), with 304 women (91.8%) reporting postpartum hair loss. The average reported time for the start, peak, and end of hair loss was 2.9, 5.1, and 8.1 months postpartum, respectively. Women with hair loss had earlier delivery times, lower birth weights, higher preterm labor rates, and longer-term breastfeeding. Multivariable logistic regression identified preterm labor and breastfeeding duration as independent predictors: compared to women who breastfed for 12 or more months, the adjusted odds ratio for postpartum hair loss was 5.96 (95% CI [1.68, 21.09]) for those stopping at 6-12 months and 6.37 (95% CI [1.95, 20.76]) for those stopping within 6 months.

Why it matters

This study provides descriptive timeline data on postpartum hair loss and identifies potential perinatal and lactation-related risk factors.

Limits

The low response rate (21.0%) creates substantial risk of participation bias, as symptomatic women may have been more likely to respond. All outcomes and clinical history were self-reported via questionnaire without objective trichological or medical confirmation, introducing recall inaccuracies. Confidence intervals for odds ratios were very wide.
