Individual Variation in the Distribution of Apolipoprotein B Levels Across the Spectrum of LDL-C or Non-HDL-C Levels.

What was done
This cross-sectional study evaluated 12,688 adult participants not taking statins from the National Health and Nutrition Examination Survey (NHANES) between 2005 and 2016. Using quantile regression, investigators analyzed the population distribution of apolipoprotein B (apoB) across the range of LDL-C and non-HDL-C levels, and quantified discordance (measured apoB minus median apoB for a given LDL-C level) across subgroups stratified by age, sex, race and ethnicity, obesity, diabetes, triglyceride levels, HbA1c, BMI, and metabolic health status (defined as BMI 18.5–24.9, triglycerides <150 mg/dL, and no diabetes).
What was found
Among 12,688 participants (median age 41.0 years, IQR 29.0–54.0; 52.9% women), LDL-C levels of 55, 70, 100, and 190 mg/dL corresponded to population median apoB values of 49, 60, 80, and 140 mg/dL, respectively. At an LDL-C level of 100 mg/dL, the 95% population distribution of apoB ranged from 66 mg/dL to 99 mg/dL. ApoB variability was highest with LDL-C calculated by the Friedewald equation, lower with Sampson or Martin-Hopkins equations, and lowest for non-HDL-C. Although individuals with metabolic risk factors had higher rates of discordantly high apoB, substantial apoB variability persisted even among metabolically healthy participants.
Why it matters
Restricting apoB measurement solely to patients with hypertriglyceridemia, as current guidelines often recommend, misses substantial atherogenic particle discordance across the general population. The percentile mappings provide reference conversions to translate standard LDL-C targets into equivalent apoB goals.
Limits
The analysis is cross-sectional and excluded statin-treated individuals, meaning findings may not apply to patients on lipid-lowering therapy. It relies on survey-derived laboratory values without tracking longitudinal cardiovascular clinical endpoints.
