A systematic review and meta-analysis of 41 studies encompassing 1,673 men with pathological gonadotropin deficiency (mean age 25 ± 5 years) evaluating reproductive outcomes after gonadotropin therapy (median duration 18 months). The authors quantified post-treatment sperm concentration, achievement of concentration thresholds (>0, >1, >5, >10, and >20 M/mL), serum testosterone, and testicular volume, comparing combined hCG/FSH versus hCG monotherapy and congenital hypogonadotropic hypogonadism (CHH) versus hypopituitarism.
What was found
The average sperm concentration achieved was 11.6 M/mL (95% CI 8.4–14.9 M/mL). Sperm concentrations >0, >1, >5, >10, and >20 M/mL were reached by 78%, 55%, 36%, 24%, and 15% of patients, respectively. Combined hCG/FSH yielded significantly greater mean sperm output and higher proportions achieving every threshold compared with hCG monotherapy. Patients with CHH had significantly lower mean sperm output and smaller testicular volume increases than patients with hypopituitarism or mixed cohorts. Increases in testosterone and testicular volume did not differ between hCG and combined hCG/FSH.
Why it matters
This meta-analysis provides clear quantitative benchmarks for spermatogenesis in hypogonadotropic hypogonadism, demonstrating that while most men achieve sperm in the ejaculate, levels typical for natural conception are reached in under a quarter of cases, and combined hCG/FSH is superior to monotherapy.
Limits
The abstract does not specify the study designs (randomized vs. observational) or risk of bias among the 41 included studies. Clinical endpoints like pregnancy and live-birth rates are not reported.
