What was done

Authors conducted a systematic review and meta-analysis comparing low-load resistance training with blood flow restriction (LL-BFR) versus high-load resistance training (HL-RT) in untrained males. Nine databases (including PubMed, Embase, Cochrane Library, Scopus, CNKI, and Wanfang) were searched through July 2024. Risk of bias was evaluated using the Cochrane tool. Standardized mean differences (SMD) were calculated for muscle strength and hypertrophy, accompanied by subgroup analyses on modulating variables such as pressure prescription, cuff inflation mode, and session volume.

What was found

HL-RT produced superior muscle strength gains compared to LL-BFR overall (SMD = -0.33, 95% CI: -0.49 to -0.18, p < 0.0001). However, subgroup analyses showed that LL-BFR achieved comparable strength gains to HL-RT when using individualized cuff pressure (SMD = -0.07, p = 0.56), intermittent cuff inflation (SMD = -0.07, p = 0.65), or higher numbers of training sessions (SMD = -0.12, p = 0.30). For muscle hypertrophy, there was no significant difference between LL-BFR and HL-RT (SMD = 0.01, p = 0.94), a finding that remained consistent across subgroup analyses.

Why it matters

This review shows that while conventional heavy lifting remains the benchmark for maximizing maximal strength, low-load BFR is equally effective for building muscle mass in untrained males. It supports LL-BFR as a viable alternative for hypertrophy when heavy loads are contraindicated due to joint pathology or injury.

Limits
