What was found

Weight loss of 10–15 kg supported durable remission of short-duration type 2 diabetes over 6 months on an isoenergetic diet and induced an immediate decrease in pancreatic fat. In the DiRECT trial, beta-cell functional capacity returned almost to normal over 12 months, and extended follow-up at 3 additional years in routine care showed a halving of serious adverse events compared with standard care. Among patients with lower baseline BMI (21–27 kg/m²), stepwise weight loss resulted in diabetes remission in 70%.

Why it matters

This work consolidates evidence that type 2 diabetes has a uniform primary aetiology—excess intra-organ fat exceeding an individual's personal threshold—making beta-cell dysfunction potentially reversible with sufficient weight loss regardless of starting BMI.

Limits

As a narrative review, it selectively highlights the author's own body of work rather than following a systematic review methodology. Numerical confidence intervals, precise participant counts across the summarized trials, and attrition rates are not reported in the abstract. Durability of remission remains strictly contingent upon avoiding weight regain.
