Antidepressant Trial Duration Versus Duration of Real-World Use: A Systematic Analysis.

This systematic review analyzed 52 placebo-controlled randomized trials (n = 10,116) evaluating the 10 most commonly prescribed antidepressants sampled at 5-year intervals from 1978 to 2023. The authors assessed trial durations and methodological characteristics (standardized scales, withdrawal monitoring, taper protocols, post-treatment outcomes) and compared them against real-world usage durations derived from the National Health and Nutrition Examination Survey (NHANES).
What was found
Median real-world antidepressant use in the United States was approximately 5 years, compared with a median clinical trial duration of 8 weeks (interquartile range: 6–12 weeks). Of the 52 trials, 88.5% (n = 46) lasted 12 weeks or less, 11.5% (n = 6) randomized participants beyond 12 weeks, and 0% lasted longer than 52 weeks. Monitoring for discontinuation effects was rare: 18.9% included taper protocols, 3.8% monitored withdrawal symptoms, and 1.9% reported post-treatment outcomes.
Why it matters
A major discordance exists between the short regulatory trial durations for antidepressants and multi-year real-world patient exposure. Clinical trials provide minimal empirical evidence regarding long-term efficacy, withdrawal symptoms, or post-treatment outcomes.
Limits
Trials were sampled at 5-year intervals rather than through a continuous, comprehensive census. NHANES duration estimates rely on self-reported survey data. The abstract does not break down results by specific antidepressant classes or clinical indications.
