The authors reviewed the limitations of standard sleep scoring criteria in neurodegenerative diseases and presented a newly proposed alternative scoring system developed by sleep centres in Bologna and Barcelona. The system adds six specific stages (Abnormal Wake, Subwake, Undifferentiated NREM, Poorly Structured N2, REM without low-amplitude mixed-frequency activity, and REM without rapid eye movements) to classify atypical neurophysiological patterns. The scoring method was tested on video-polysomnography recordings from nine patients with Parkinson's disease, dementia with Lewy bodies, and multiple system atrophy to evaluate interrater reliability.
