Mystical experiences during magnesium-Ibogaine are associated with improvements in PTSD symptoms in veterans.

What was done
Thirty male military veterans with traumatic brain injury from repeated blast or combat exposures underwent magnesium-ibogaine therapy. Mystical experiences were assessed post-treatment using the 30-item Mystical Experiences Questionnaire (MEQ30). Resting-state electroencephalography (EEG) and PTSD severity were measured at baseline, immediately post-treatment, and one month post-treatment. Linear mixed models tested the interaction between time and MEQ30 scores on clinical and neurophysiological outcomes.
What was found
Greater mystical experience intensity was significantly associated with larger reductions in PTSD symptoms from baseline immediately post-treatment (adjusted B = -5.89, adjusted p < 0.001) and at 1 month post-treatment (adjusted B = -4.45, adjusted p = 0.007). Higher MEQ30 scores were also associated with greater decreases in resting-state EEG peak alpha frequency at one month (adjusted B = -0.38, adjusted p = 0.006).
Why it matters
These findings suggest that subjective mystical experiences may contribute to the therapeutic efficacy of ibogaine for combat-related PTSD, alongside lasting changes in resting-state brain activity.
Limits
The study is constrained by a small sample size (n = 30), a non-randomized single-arm design without a sham or placebo comparator, an exclusively male veteran cohort, and a short 1-month follow-up window.
