The effect of blood flow restricted exercise on measures of health and physical fitness across all populations: An umbrella review and meta-meta-analysis.

What was done
This preregistered umbrella review followed PRISMA guidelines to synthesize meta-analyses evaluating blood flow restricted exercise (BFRE; aerobic, resistance, or combined) compared to exercising and non-exercising control conditions across all populations. Five databases were searched, risk of bias was assessed using AMSTAR-2, and a multilevel meta-analysis of standardized mean differences (SMDs) was conducted alongside subgroup analyses.
What was found
Across 47 meta-analyses comprising 265 unique studies: - Compared to low-load resistance training, BFRE improved hypertrophy (SMD = 0.39, p < 0.001) and strength (SMD = 0.61, p < 0.001). - Compared to high-load resistance training, BFRE had no significant difference for hypertrophy (SMD = -0.13, p = 0.142) and smaller strength gains (SMD = -0.28, p < 0.001). - BFRE showed small-to-moderate improvements in aerobic fitness (SMD = 0.50, p < 0.001), vascular health (SMD = 0.45, p < 0.001), blood pressure (SMD = 0.46, p < 0.001), and muscular power (SMD = 0.56, p < 0.001). - BFRE had no significant effect on physical function (SMD = 0.16, p = 0.096), pain (SMD = 0.00, p = 0.996), or speed (SMD = 0.22, p = 0.213).
Why it matters
This overview consolidates evidence showing BFRE is an effective alternative to low-load exercise for muscle, aerobic, and vascular adaptations, though high-load resistance training remains superior for maximal strength development.
Limits
All 47 included meta-analyses were rated as low-to-moderate quality via AMSTAR-2. Total individual participant sample size was not reported in the abstract, and potential primary study overlap across meta-analyses was not detailed in the abstract text.
