Neural Correlates of Ibogaine: Evidence From Functional Neuroimaging of Military Veterans.

Researchers conducted an open-label, longitudinal neuroimaging study in 30 male Special Operations Forces veterans with sequelae of traumatic brain injury (TBI) who received ibogaine treatment. Participants underwent multimodal functional neuroimaging—specifically arterial spin labeling (ASL) and blood oxygen level-dependent (BOLD) functional magnetic resonance imaging (fMRI)—before treatment, immediately post-treatment, and at a 1-month follow-up. Whole-brain exploratory analyses evaluated resting-state regional cerebral blood flow (rCBF) and functional connectivity.
What was found
Treatment was followed by progressive increases in rCBF across cortical, limbic, and striatal subregions, alongside functional connectivity alterations across several functional brain networks. Increases in rCBF in the left insula and left anterior cingulate cortex correlated significantly with improvements in TBI-related disability symptoms. The abstract did not report specific numerical values, effect sizes, or exact p-values.
Why it matters
This study provides initial human functional neuroimaging evidence suggesting that ibogaine induces sustained changes in paralimbic cerebral blood flow and widespread network reorganization that track clinical symptom recovery.
Limits
The study is limited by a small sample size (n = 30) consisting entirely of male military special operations veterans, limiting generalizability. There was no control or placebo group, preventing causal attribution of the neuroimaging or clinical changes directly to ibogaine versus placebo effects, expectation, or non-specific time effects. Furthermore, the neuroimaging analyses were exploratory.
