mRNA-based COVID-19 vaccination and risk of autoimmune diseases in the pediatric population.

What was done
Analyzed electronic health record data from the TriNetX US collaborative network for children aged 0–21 years from April 2020 to April 2025, alongside overall incidence trends from 2016 to 2024. Using a retrospective matched cohort design, researchers evaluated incident autoimmune diagnoses over a fixed 180-day follow-up period across three comparisons: (1) vaccinated versus unvaccinated children (n = 936,919 vs. 942,604), (2) vaccinated versus unvaccinated children with prior SARS-CoV-2 infection (n = 44,512 per group), and (3) vaccinated children with versus without prior infection (n = 43,170 per group). Hazard ratios (HRs) were estimated using Cox proportional hazards models.
What was found
Pediatric autoimmune disease incidence increased beginning in 2021. In the matched cohort analyses: (1) vaccinated children had a significantly lower hazard of autoimmune disease over 180 days compared to unvaccinated children (HR = 0.387; 95% CI: 0.365–0.410); (2) among children with prior SARS-CoV-2 infection, vaccination was associated with a lower hazard compared to non-vaccination (HR = 0.690; 95% CI: 0.516–0.922); and (3) no significant difference was observed between vaccinated children with versus without prior infection.
Why it matters
These findings provide large-scale epidemiological evidence that mRNA COVID-19 vaccination does not increase short-term risk of autoimmune diseases in children, observing an inverse association even in those with previous SARS-CoV-2 infection.
Limits
The retrospective observational design cannot eliminate residual confounding or healthy vaccinee bias, wherein healthcare-seeking behavior or baseline health status differ between groups. Follow-up was restricted to 180 days, precluding evaluation of longer-term autoimmune sequelae. Specific autoimmune conditions were aggregated into an all-condition composite in the reported abstract, and EHR data may contain diagnostic misclassification or missing off-network events.
