Exercise snacks performed in real-world settings reduce postprandial hyperglycaemia and glycaemic variability in individuals living with type 2 diabetes: a randomised crossover study.

What was done
Thirty-one inactive adults with well-controlled, non-insulin-treated type 2 diabetes (21 female, 10 male; approximately 58 years old, BMI ~31 kg/m², HbA1c ~48 mmol/mol [6.6%]) completed a randomized, counterbalanced crossover trial in a real-world setting under a standardized diet. Participants completed two 48-hour conditions: an exercise snack condition (ES; four 1-minute bouts of vigorous bodyweight exercise per day on 2 consecutive days, guided by video) and a control condition (CON; 2 consecutive days of no exercise). Continuous glucose monitors and smartwatches assessed glucose regulation and heart rate. The primary outcome was 48-hour mean glucose; secondary outcomes were glycemic variability markers and 2-hour postprandial glucose metrics.
What was found
The primary outcome of 48-hour mean glucose did not significantly differ between ES and CON (difference -0.2 mmol/l; 95% CI -0.4, 0.0; p=0.07). However, ES produced small but statistically significant improvements versus CON in standard deviation (-0.1 mmol/l; 95% CI -0.2, -0.1; p<0.001), coefficient of variation (-1%; 95% CI -2, 0; p=0.007), mean amplitude of glycemic excursions (-0.3 mmol/l; 95% CI -0.5, 0.0; p=0.04), and time in tight range (3%; 95% CI 0, 6; p=0.04). Postprandial 2-hour glucose average, peak, area under the curve, and incremental area under the curve were also significantly lower after breakfast and dinner during ES (all p<0.05).
Why it matters
This study shows that totaling just 4 minutes per day of vigorous bodyweight exercise can acutely reduce glycemic variability and postmeal glucose spikes in real-world settings among adults with type 2 diabetes.
Limits
The sample size was small (n=31), and the intervention duration was very short (2 days per condition), leaving long-term adherence, safety, and sustained glycemic impact unknown. The population was limited to inactive individuals with well-controlled, non-insulin-treated diabetes. The primary outcome of mean 48-hour glucose did not achieve statistical significance.
