What was done

Authors conducted a scoping review following PRISMA guidelines to evaluate peer-reviewed evidence for six emerging peptides (BPC-157, thymosin beta-4/TB-500, CJC-1295, MK-677, ipamorelin, and GHK-Cu) for musculoskeletal recovery, tissue healing, or performance enhancement. Databases were screened independently by multiple reviewers for studies involving either animal or human models.

What was found

Preclinical animal models (primarily rats) comprised 67% of all identified publications, showing mechanism-level promise but variable effects on bone, tendon, muscle, and ligament healing. Human evidence was sparse, heterogeneous, and mostly derived from studies lacking robust controls or rigorous methodology. Reported human benefits were modest at best and limited to degenerative knee pain and metabolic bone health. Significant safety concerns were identified, including risks of congestive heart failure with MK-677, insulin resistance, and wide variations in dosing and administration routes.

Why it matters

Despite aggressive marketing and widespread adoption in sports medicine and fitness communities, evidence supporting therapeutic peptide use in humans is largely absent, while clinically relevant adverse effects exist.

Limits

The total number of included studies and participants was not reported in the abstract. Most underlying data rely on rodent models that cannot be directly translated to human outcomes, and available human studies lacked standardized dosing, rigorous control groups, and robust design.
